What it means
Creating a user, or changing a user's email, to an address another user of the same environment already has. On the account API, an email change whose address somebody else took between the request and the confirmation; the confirmation link is then spent.
What to do
Use another address. Addresses are unique within one environment, never across environments. After a spent confirmation, ask for the change again.
